Informed Consent/Privacy Practices
(Please note, recreational therapist and practitioner are used interchangeably throughout this document.)

Please read through the following informed consent agreement. What follows is a basic understanding between the client and the recreational therapist. What is listed below are the responsibilities and obligations of your recreational therapist and some expectations of you, as the client. This document also contains important information about our professional services and business policies.
Recreation therapy consists of assessment and activity-based interventions to assist with the various domains of well-being. Those domains may include physical, social, emotional, cognitive, and mental health.
The client should know their rights, as well as, the responsibilities for participation and Recreation Therapy. This informed consent should be able to explain the scope and limitations of this therapy including the rights and responsibilities of the parties to therapy

About the therapist:
Laura is a licensed recreational therapist, a certified therapeutic recreation specialist, and a certified brain injury specialist. She specializes in working with survivors of Acquired and traumatic brain injuries, and other neurological disorders. She also enjoys working with folks trying to manage their anxiety and depression especially as it relates to the early stages of Parenthood. She has been practicing since 2012 and has multiple experiences in inpatient psychiatric care, patient physical rehabilitation, community-based programming, and case management.

Risks and expectations:
Recreation therapy is a unique therapy that not only explores one's self-awareness, potential painful history, and potential emotions but can also be active in nature. It is the client's responsibility to inform the recreation therapist of any discomfort or pain. Please let your Recreation Therapist know of any physical restrictions prescribed by your doctor.
It is the client's responsibility to inform the recreational therapist of any updates to their physical, cognitive, emotional, and social status. Recreate Yourself, LLC provides recreational therapy based in the community. Recreate Yourself, LLC is not responsible for any injuries that occur due to the neglect of the community setting.
The Health Insurance Portability and Accountability Act:
HIPAA is a federal law that regulates the disclosure or distribution of patient information by Healthcare Providers to others.
This information is called protective health information and cannot be transferred or used by Healthcare professionals without prior written consent from the clients. For PHI to be considered as such it must be informed that they have received or that they have created through a course of the practice of Business by a Healthcare provider, Healthcare plan, or Healthcare Clearing House. It must be information relating to medical or mental health condition, payments or diagnoses or provision of healthcare providers or professionals whether it be related to past, present, or future events or information relating to the individual and lastly, it must identify the patient or individual.
In case the information of a patient is going to be used for research, they must inform the institutional review boards, as well as the patient who owns the information
Confidentiality agreement:
Any personal information shared with Recreate Yourself, LLC is considered confidential information covered by HIPAA. We cannot disclose information to anyone without obtaining verbal or written consent from you. Please note that this confidentiality agreement between the client and the practitioner is not absolute, here are some limitations:
-if your practitioner has a good reason to believe that you will harm yourself or another person
-if your practitioner has a good reason to believe you are abusing a child or vulnerable adult
-if you are committing sexual abuse
Group Sessions:
Your practitioner may recommend participation in a group setting. Your practitioner will share the least confidential information possible with group members and community affiliates, however, these same standards do not protect confidentiality from other group members. We cannot control what information other group members may disclose. Your confidentiality cannot be guaranteed in group therapy. Therefore, confidentiality within group therapy is a shared responsibility among all members and is based on mutual trust and respect.
Cancellation Policy:
Sessions are scheduled by appointment only. In case you have a scheduled appointment and you wish to cancel, please give us a 24-hour notice to not be charged a cancellation fee. Your practitioner has the right to cancel or reschedule your session if needed but will do their best to keep the scheduled times.
Payment Policy:
Payment is due at the time of service. Recreate Yourself, LLC does not accept insurance at this time. You are responsible for paying the bill in full unless special arrangements have been made in advance. Late fees of 1.5 % per month will be charged on balances that are still unpaid starting 30 days after the first statement. There is a fee of $20 for returned checks. Delinquent accounts may be turned over to a collection agency at which time you agree to be responsible for collection charges and all associated legal fees in addition to the amount owed. We accept cash, debit, credit cards, and FSA/HSA cards (you are responsible for verifying FSA/HSA acceptance with your cardholder).
